This resource provides templates you can fill out to keep track of your cancer care history. In the Cancer Treatment Plan template you can keep track of treatments you received, medications, surgeries, etc. so that you can easily share with your health care team. It also includes a Survivorship Care Plan template where you can organize information about your recommended follow-up care plan. You can download and use these for any type of cancer. It also includes a specific Survivorship Care Plan template for breast cancer, cervical cancer, lung cancer (NSCLC or SCLC), diffuse large B-cell lymphoma, ovarian cancer, prostate cancer, or uterine/endometrial cancer.
